Your Path to Improved Health

We all want to live a healthier lifestyle, but finding the time to invest in your health can be difficult. Or, you’re really committed to getting healthier, but you’re not sure where to start. With the health and wellness services offered by JET Physical Therapy, you can get the help you need to improve your health, fitness and diet with a plan specifically for you.

Gateway to Health

The foundation of our program is Gateway to Health. Each Gateway to Health patient undergoes a 40-minute health and movement screening and a discussion of your goals, which may include:

  • Improved strength and fitness
  • Improved mobility and balance
  • Pain relief
  • Weight loss
  • Improved sports performance
  • Physical assessment
  • Exercise planning, including which exercises to perform and guidance on how often to perform them
  • One weekly session of exercise with your health coach at JET Physical Therapy facilities
  • Personal instruction on how to perform exercises safely

Healthy Living Link

Good nutrition is the foundation of any wellness plan. You can exercise every day, but you likely won’t see the results you want if you don’t improve your nutrition habits. With Healthy Living Link, you receive a combination of group support and personal coaching to learn techniques to improve your nutrition over the course of a 16-week program.

Nutrition Improvement Elements

Your health coach will sit down with you for a discussion and have you fill out a questionnaire at the beginning of the program. Based on that information, a nutrition plan will be developed for you to include:

  • A balanced meal plan
  • Weekly food journaling
  • Nutrition education
  • Access to a registered dietician (as available)
  • Grocery store tour (as available)

Group Support Sessions Include:

  • 7 one-hour group education classes
  • Discussions about healthy living topics
  • Support from other individuals in the program

Personal Coaching Includes

  • Weekly private sessions with your health coach
  • One hour of health coaching and physical activity guidance each week
  • Discussion to review your challenges, successes and meal plan
  • Accountability coaching to ensure you are progressing toward your goals
